This page tells you four things: how to get a copy of your records, how to reach the board that licenses me, how to file a complaint if you have one, and complaining to me directly. You do not need my permission to use any of it, and using it will not affect the care you receive.

1. How to request your health care records

Put your request in writing. A signed letter or an email is fine. Include:
Your full name and date of birthThe dates or range of dates you want coveredWhat you want, whether that is the full record, a summary, or specific documentsWhether you want it electronically or on paperWhere to send it, and a phone number in case I have a questionYour signature and the date
Send it to:
Keely Rodriguez, LPC-S Keely Rodriguez Therapy 1623 Central Blvd, 2nd Floor, Suite 1, Brownsville, Texas 78520 keely@keelyrodrigueztherapy.com (956) 275-3232
How long it takes. If you ask in writing for your record in electronic form and I maintain it in a system that can produce it that way, Texas law gives me 15 business days. Texas Health and Safety Code Section 181.102. For records in other formats, I will provide them promptly and in no case later than 30 days.
What it costs. There is no charge for your first request. I will verify your identity before releasing records, and I will not delay a request over a fee question.
If I say no. In narrow circumstances the law lets a mental health professional withhold part of a record, generally when release would be harmful to you or would identify another person. If that happens I will tell you in writing which part I am withholding and why, and I will offer to send the record to another licensed professional of your choosing instead.
If you want records sent somewhere else. Records go to a third party only with your written authorization. I will give you the form.

2. How to contact the board that licenses me

I am licensed by the Texas Behavioral Health Executive Council, which houses the Texas State Board of Examiners of Professional Counselors.
Contact: Keely Rodriguez, LPC-S Licensed Professional Counselor-S, Texas License No. 78535
Texas Behavioral Health Executive Council 1801 Congress Avenue, Suite 7.300 Austin, Texas 78701 Phone: (512) 305-7700 or 1-800-821-3205. Website: bhec.texas.gov
You can verify any Texas behavioral health license, and see whether a licensee has been disciplined, on the Council's website.
To file a complaint about a counselor, including me, go to bhec.texas.gov and follow the complaints and enforcement instructions, or call the number above and ask for the enforcement division. You do not need a lawyer and you do not need my permission. Complaints can be submitted online or by mail.

3. How to file a consumer complaint about health information privacy

If you believe your health information has been handled improperly, you can file a complaint with the Texas Attorney General, which enforces the Texas Medical Records Privacy Act.
Office of the Attorney General of Texas Consumer Protection Division Website: texasattorneygeneral.gov/consumer-protection/health-care/patient-privacy Consumer Protection Hotline: (800) 621-0508
The Attorney General maintains a website explaining your privacy rights, listing the state agencies that regulate health care providers, and describing each agency's complaint process. Texas Health and Safety Code Section 181.103.

How to file a complaint with the Texas Behavorial Health Executive Council

The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.

Complaining to me directly

You are also welcome to bring a problem straight to me. Email keely@keelyrodrigueztherapy.com or call (956) 275-3232.
I would rather hear it than not. But going to me first is not a requirement, and nothing about raising a concern with me limits your right to go to the Council or the Attorney General instead, or at the same time.
You will not be retaliated against for filing a complaint, requesting your records, or asking a question about your rights.
